The information and links contained below are for health professionals who currently refer patients to the Huntingdonshire Exercise Referral Scheme or who wish to register to refer patients.

How to get registered?

Only registered Health Professionals may make referrals. Health Professionals may be a: General Practitioner, Practice Nurse, Physiotherapist or Occupational Therapist

How do I refer a patient?

You can use the link on the right named Exercise Referral Form to refer a patient. All information should be completed as fully as possible.

Who is eligible?

All residents of Huntingdonshire, aged 16 years or above, whose Health Professional is registered on the Exercise Referral Scheme
In certain instances, residents of Huntingdonshire, aged 16 years or under, whose Health Professional is registered on the Exercise Referral Scheme. Please call Exercise Referral Team to discuss on 01480 388469
All Cardiac Rehabilitation patients from Papworth Hospital, Hinchingbrooke Hospital & Addenbrookes Hospital
The Scheme welcomes individuals with any medical condition that may benefit from controlled, regular exercise subject to the availability of appropriately qualified staff.

Target groups include:

Cardiac Rehabilitation
>1 Coronary Heart Disease Risk Factor
Diabetes (Type 1 & 2)
Hypertension
Mental Health (Depression/Anxiety)
Obesity
Respiratory Conditions (including Asthma)

What is the cost?

There is no cost to the Referring Agency or Health Professional
£10.00 for the patient to enrol on to the Exercise Referral Scheme. This includes lifetime membership of the Fitness Suite, both the initial and final Consultations/Fitness Assessments. A further £10 deposit is required for a Wellness exercise programme key.
Each exercise session within the twelve-week referral period is £2.80. The duration and frequency of sessions will vary from individual to individual, but between 30-60 minutes twice to three times per week is normal
